Coaches Pledge *- Filled out by Head Coach and submitted to Commissioner. Goes in packet submitted by Commissioner to AA County. For Boys and Girls Head Coaches.

Commissioner Form - Boys *- Filled out and submitted w/ payment by Boys Commissioner.

Commissioner Form - Girls *- Filled out and submitted w/ payment by Girls Commissioner

Concussion Awareness Form - Boys and Girls *- Filled out by parent, submitted to Head Coach. Head Coach submits to Commissioner for KIYL records.

KIYL COVID-19 Action Plan *

Player Release Form *- Filled out by parents of children who previous played in another AA County participating team last year. Submitted to Head Coach. This document is only necessary if you played in another Anne Arundel Rec team last year.

QAC COVID-19 Return to Play Guidelines *- QAC COVID-19 Return to Play Guidelines

Team Registration Form - Boys *- Filled out jointly by Head Coach and Boys Commissioner. One form per team. Submitted to AA County by Boys Commissioner.

Team Registration Form - Girls *- Filled out jointly by Head Coach and Girls Commissioner. One form per team. Submitted to AA County by Girls Commissioner.

Team Roster Form *- Filled out by Head Coach and submitted to Commissioner. Goes in Commissioner packet for AA County. For Boys and Girls teams.

Youth Player Contract - Boys and Girls *- Filled out by parent, submitted to Head Coach. Part of packet Commissioners submit to AA County.
